8、-systolic volume=125ml-55ml=70ml,（2）射血分数（Ejection fraction,EF）：搏出量和心室舒末容积的百分比。Ejection Fraction is the ratio比值 of SV 搏出量to end-diastolic volume 舒末容积 EDC 舒末容积（end-diastolic volume,EDV）about 110 to 120 ml for normal adult 缩末容积（end-systolic volume,ESV）.EF=SV/EDV 100%(55-60%for the health)if SV=70ml，EDV

9、=140ml EF=70m l/140ml=50%,2.每分输出量Minute volume和心指数cardiac index(1)心输出量（Cardiac output，CO）：每分钟由一侧心室输出的血量称为每分输出量，也称心输出量。Cardiac output is the quantity of blood pumped into the arteries by heart each minute 心输出量=搏出量 心率 CO=SVHR(heart Rate)Minute volume=SVHeart Rate=(60 80ml)75？左心输出量 右心输出量,(2)心指数（cardiac

11、SV ejection pressure+kinetic energy动能左室搏功（J）=搏出量（L）（平均动脉压-平均左心房压mmHg）（13.6g/cm3）9.807（1/1000）ejection pressure=MAP LAMP(mmHg)右室为左室的1/6 每分功=每搏功HR Minute work=SW heart Rate(HR)意义:,三、影响心泵功能的因素 CO=SV HR,（一）前负荷,异长自身调节(heterometric autoregulation)心肌的前负荷(心室舒末容积或压力，取决于回心血量)Venous return determines the prelo

12、ad of heart on end-diastolic volume.Therefore,end-diastolic volume or end-diastolic pressure is considered as preload of heart.,1.前负荷对搏出量的影响 心泵功能的自身调节-the Frank Starling 机制 intrinsic自身的 regulation of heart pumping the Frank Starling mechanism 在没有神经体液因素参与的情况下心输出量是静脉回流量的函数。,心脏前负荷与肌肉前负荷之相同与不同点 心功能曲线 Ve

13、ntricular function curves：以左室搏功对左室舒张末期压力作图，得到的曲线称为心功能曲线 特点是心功能曲线无降支,横轴可以是左心房压中心静脉压纵轴可以是搏出量缩短速度心输出量每搏功率,异长自身调节定义：通过改变心肌细胞初长度而引起 心肌收缩强度变化的适应性反应，称为异长自 身调节机制：a.前负荷决定初长度，从而影响可活化横桥 的数目 b.存在肌钙蛋白长度依赖性钙敏感度的变化 c.心肌伸展性小，故而无降支意义：有利于维持正常泵血功能 心输出量是静脉回流量的函数（Starling心定律）,2.影响前负荷的因素 心室舒末容积=余血量+静脉回流量（生理条件下 是主要因素）静脉回流

14、量以下因素的影响：（1）心室舒张充盈持续时间（2）静脉回流速度（3）心包内压：病理情况下心包积液（4）心室顺应性（V/P）,（二）后负荷1.后负荷对搏出量的影响After-load and its effect on heart pump 大动脉血压相当于后负荷 After-load is the arterial pressure.对泵血过程的影响 等容收缩期延长 射血时间延迟并缩短 射血速度减慢 搏出量减少,整体条件下后负荷对搏出量（或心输出量）的 影响(在很大范围内不影响心输出量,但做功量增加)现象：,（三）心肌收缩能力的改变对搏出量的调节 The effect of heart con

15、tractility on heart pump 心肌收缩能力（contractility，也称等长调节homometric regulation）：心肌不依赖于负荷而能改变其力学活动的内在特性，称为心肌收缩能力。,（四）心率对心泵功能的影响 可以在40200次/分变动 Within the physiologic ranges of heart rate(from 60 beats/min to 180 beats/min for the top endurance有耐力的 athletes运动员),minute cardiac output increases as the heart rate increases,心输出量=搏出量心率 若搏出量不变，心率加快则心输出量减少 但快到某种程度则心动周期过短（主要是舒张期缩短）心室充盈量减少，所以搏出量减少，心输出量不会继续增加：心率过慢时并不能使充盈量增加,
