17、工业贵州质量监督中心站申请,主要内容应包括:准予建设的程序性文件、初步设计审批、投产标准、工程建设概况、参建各方责任主体质量行为概况、单位工程质量认证情况等)。2、矿井初步设计及安全专篇审批文件(复印件应注明原件存放处并加盖其公章)。3、已认证单位工程明细表和未认证单位工程明细表(由矿区站确认)。4、设计、勘察单位出具的单项工程质量评价报告。5、监理单位出具的单项工程质量评价报告。三、认证程序(一)中心站收到单项工程质量认证申请报告后,首先审查是否具备以下认证条件: 1、工程项目各种审批手续齐全。2、主要生产系统已经形成。3、竣工单位工程个数达到设计总数的85%以上,且均已通过单位工程质量认证
18、。4、工程质量监督费按规定全部缴纳。(二)中心站在对认证条件审查合格后,组织单项工程质量认证专家组(以下简称认证组),进行单项工程质量认证。认证组一般分为矿建、土建、机电安装及综合等专业组,且每个专业组不少于3人。(三)认证组听取参建各方质量责任主体的工程建设情况及矿区站对所含单位工程质量认证情况的介绍,并进行质询。(四)确定抽查的单位工程,其中抽查各类单位工程个数分别不少于已认证单位工程个数的15%,抽查对象要兼顾主要工程与一般工程、优良工程与合格工程、施工承包单位和分包单位。(五)核定抽查的单位工程质量等级,评定单项工程质量等级,形成专业组认证意见和单项工程质量认证意见,并向各方责任主体书
19、面宣布。(六)建设单位负责组织参建各方责任主体,对认证组提出的问题进行整改,并将整改情况书面报矿区站,由矿区站对整改情况复核合格后,书面中心站。(七)中心站出具单项工程质量认证书。认证书一式6份,总站存档(由中心站出具的单项工程质量认证书,中心站须报总站备案)1份、中心站存档1份、矿区站存档2份,发给建设单位2份。四、办理程序1、煤炭工业贵州质量监督中心站审查受理;2、现场认证;3、出具单项工程质量认证书。煤炭工业建设工程质量监督工作规则第一章 工程质量监督注册第一条 凡属煤炭工业建设工程项目,建设单位必须在开工前到质量监督站办理质量监督注册手续,填写单位工程、单项工程质量监督注册申报表,并按
20、规定交纳工程质量监督费。第二条 建设单位办理工程质量监督注册手续时,要向质量监督站提供以下资料:一、单项工程1、项目准予建设的程序性文件。2、设计、勘察、施工、监理等企业中标通知书、资质证书及工程合同。3、建设、设计、勘察、施工、监理等单位的工程项目负责人和机构组成(建设、设计勘察、施工、监理等单位的工程质量保证体系表)。4、监理规划。5、单位工程名细。6、其它需要的有关文件资料。二、单位工程(不属于单项工程范围的单位工程)1、设计、勘察、施工、监理等企业中标通知书、资质证书及工程合同。2、建设、设计、勘察、施工、监理等单位的工程项目负责人和机构组成。3、施工组织设计和监理规划。4、其它规定所
21、需要的有关文件资料。第三条 在工程项目施工过程中,若施工、监理等企业发生变化时,应及时向质量监督站提供变化后的中标通知书、企业资质证书、工程合同以及工程项目负责人和机构组成等有关资料。第四条 质量监督站应在7个工作日内审核完毕,符合规定的发给煤炭工业建设工程质量监督书。第五条 建设单位凭煤炭工业建设工程质量监督书向有关管理部门办理工程项目开工手续。未办理工程质量监督注册手续的,不得开工。第二章 工程质量监督方案第六条 对于单项工程或大型单位工程,质量监督站应制定工程质量监督方案;对于单位工程,按建设工程质量监督计划书的要求编制工程质量监督计划。质量监督站要根据工程项目进展情况,择期召集责任主体
22、有关人员进行质量监督交底,工程质量监督计划应在工程项目质量监督注册后7个工作日内以书面形式通知建设、设计、勘察、施工、监理等单位。第七条 质量监督站应根据工程项目初步设计或施工组织设计、监理规划或监理实施细则,在工程质量监督方案或监督计划中明确以下重点:一、监督检查责任主体和有关机构质量行为的具体内容和方式。二、对各专业工程实体质量监督检查(包括监督检测)的具体内容、方式、重点。三、工程竣工验收内容。第三章对工程参建各方主体质量行为的监督第八条 质量监督站对责任主体和有关机构的质量行为进行监督应遵守以下一般规定:一、抽查责任主体和有关机构执行有关法律法规及工程技术标准的情况。二、抽查责任主体和
