4、roliferation of vessel wall in the group of hypoxemic group were higher than those in normal group（P0.01）.There was no expression of CTGF protein in the pulmonary arteriole of the normal group，whereas，it was highly expressed in the tunica adventitia，slightly expressed in the endangium and rarely exp

6、ary vascular remodeling;connective tissue growth factor 摘要目的: 探讨新生儿感染性疾病早期诊断的方法和降钙素原（PCT）在新生儿感染性疾病诊断中的临床意义。 方法: 对76例疑似新生儿感染患儿，在应用抗生素前测定PCT、C反应蛋白（CRP）、血常规及行血培养，比较它们对细菌感染诊断的敏感性、特异性。 结果: （1）血清PCT检测对细菌感染诊断的敏感性为81.36%，特异性为64.71%。（2）CRP对细菌感染诊断敏感性为47.46%，特异性为70.59%;与血清PCT检测结果比较，敏感性有显着差异（P0.01），特异性无显着差异（P0.

7、05）。（3）白细胞计数或（和）分类异常升高对细菌感染诊断敏感性为52.54%，特异性为52.94%，与血清PCT检测结果比较有显着差异（P0.05）。 结论: 血清PCT可作为新生儿感染的早期检测指标，其作用优于CRP测定和外周血白细胞计数及分类。通过对PCT水平的动态观察，既可早期诊断、及时治疗新生儿感染性疾病，又可判断感染是否被控制。 关键词 降钙素原;新生儿感染;早期诊断 目前，在我国新生儿危重症监护病房中，细菌感染性疾病尤其是全身性细菌感染性疾病仍占相当比例，而且病死率较高。此类疾病的早期诊断和及时治疗对降低新生儿病死率具有积极意义，但目前尚缺少一种快速可靠的实验室诊断方法，以致患儿

8、在重症细菌感染时，实验室指标未出现阳性结果，病情已有恶化，丧失抢救的最佳时机。新近发现，血清降钙素原（PCT）的升高与细菌感染密切相关1 ，PCT被认为是诊断全身细菌感染性疾病的重要标志物。有鉴于此，作者对76例疑似新生儿感染患儿进行血清PCT的检测，并与C反应蛋白（CRP）、外周血白细胞检测结果进行比较，以探讨PCT在新生儿感染性疾病中的诊断价值。 1 对象与方法 1.1 研究对象 2003年3月2004年8月，在本院新生儿科住院怀疑有全身感染性疾病的新生儿76例，男45例，女31例，平均年龄13.24d（028d）。76例患儿按出院（或死亡）诊断分为两组:细菌感染组59例，其中败血症13例

9、，肺炎19例，腹膜炎2例，脐炎15例，脓疱病4例，4S综合征1例，肠炎5例;非细菌感染组17例，其中缺氧缺血性脑病8例，母乳性黄疸6例，ABO溶血症3例。 1.2 血清PCT、CRP检测方法 新生儿入院后，在应用抗生素前采集静脉血，测定PCT、CRP及血常规，同时进行细菌培养。血清PCT检测采用半定量固相免疫测定法，PCT.Q检测卡为德国BRAHMS公司产品（由上海欧典生物技术有限公司提供），按说明书操作，检测结果分为0.5ngțml-1 ，0.52.0ngțml-1 ，2.110.0ngțml-1 ，和10.0ngțml-1 4个等级，正常值为0.1n

10、gțml-1 ，按文献报道，PCT0.5ngțml-1 为阳性阈值。CRP采用免疫比浊法（芬兰Orion Diagnostica公司生产的CRP.Quickread仪），8mgțL-1 为阳性，8mgțL-1 为阴性。 1.3 分析方法 比较细菌感染和非细菌感染两组患儿血清PCT的检测结果，并按文献提供的方法2 计算出该试验对细菌感染诊断的敏感性和特异性，同时与CRP、外周血白细胞计数和分类结果进行比较。组间差异采用2 检验或u检验。 2 结 果 2.1 血清PCT检测结果 见表1。表1 两组患儿血清PCT检测结果比（略）2.2 CRP检测结果 细菌感染组59例中，CRP阳性28例，占47.46%;非细菌感染组17例中，CRP阳性5例，占29.41%。CRP对细菌感染诊断敏感性为47.46%（28.59），特异性为70.59%（.17）;与血清PCT检测结果比较，对细菌感染诊断敏感性有显着差异（P0.01），特异性无显着差异（P0.05）。
